Background

Gluteus medius (GM) tears frequently coexist with hip osteoarthritis. This study evaluates outcomes of total hip arthroplasty (THA) with concurrent GM repair via the direct anterior approach (DAA).

Methods

14 female patients underwent THA with GM repair using the DAA, compared to a matched control cohort undergoing THA without GM repair.

Key Findings

  • Patients showed significant improvements in PROMs at 2 years.
  • Outcomes and patient satisfaction were comparable to THA patients without GM pathology.

Conclusions

THA with simultaneous GM repair using the DAA provides favorable functional outcomes similar to isolated THA.

What Does This Mean for Providers

  • The DAA facilitates simultaneous treatment of gluteus medius pathology during THA.
  • Addressing GM tears at the time of THA may improve hip stability and functional outcomes.
  • This combined approach is a viable surgical option for patients with coexisting hip arthritis and GM tears, with outcomes comparable to standard THA.
